The survey can be viewed as a Microsoft PowerPoint Presentation or as a Web Page.

PowePoint Survey    (Requires that Powerpoint or Viewer is installed on your pc)

If the PowerPoint Survey does not work than you may need to install PowerPoint Viewer first.

Install PowerPoint Viewer

Answer "Run" to install PowerPoint Viewer if "Run" is a choice.

Answer "Save" to download PowerPoint Viewer (ppviewer.exe) to your pc if "Run" is not a choice.
Install the PowePoint Viewer by double clicking on the downloaded ppviewer.exe file.
